The Waukesha Employee Health & Wellness Center is open to the following County individuals:
Employees enrolled in a County medical insurance plan
Dependents, including spouses and children over the age of 2*, who are enrolled in a County medical insurance plan
*The minimum age is 2+ for standard clinic services and 12+ for physical therapy services.
All County employees are eligible for worker's compensation services provided in the Health & Wellness Center.
Please Note: Payment will be collected at time of service via a credit or debit card. Cash/check not accepted.
[1] Due to IRS regulations, HSA eligible plan members must pay fair market value for visit. All fees apply towards deductible and out of pocket maximum. Once deductible is met, non-preventive visits and physical therapy visits are free for those on the High Deductible Health Plan.

Program Dates: February 1 - September 30
The County Health Screening Program (formerly HRA Program) is voluntary and consists of completion of a biometric screening (which includes blood draw, height, weight, and blood pressure) to earn the health plan incentive. Employees and spouses on the health plan will be assigned a month to complete their appointment at the Waukesha Employee Health & Wellness Center based on the first letter of the employee's last name.

The County Wellness Rewards Program is a voluntary program allows eligible employees to participate in various wellness activities to accumulate points, which can be redeemed for reward money. For more information, click ‘View Incentives’ in the My Incentive Status box on the home page of your patient portal.
