Eligibility
The Waukesha Employee Health & Wellness Center is open to the following City individuals:
Employees enrolled in a City medical insurance plan
Pre-Medicare / Active retirees enrolled in the City's UMR insurance plan
Dependents, including spouses and children*, who are enrolled in a City medical insurance plan
*The minimum age is 2+ for standard clinic services and 12+ for physical therapy services.
All City employees are eligible for worker's compensation services provided in the Health & Wellness Center.
Cost
Please Note: Payment for medications will be collected at time of service via a credit or debit card. Cash/check not accepted.
