City of Waukesha

The Waukesha Employee Health & Wellness Center is open to the following City individuals:

• Employees enrolled in a City medical insurance plan

• Pre-Medicare / Active retirees enrolled in the City's UMR insurance plan

• Dependents, including spouses and children*, who are enrolled in a City medical insurance plan

*The minimum age is 2+ for standard clinic services and 12+ for physical therapy services.

All City employees are eligible for worker's compensation services provided in the Health & Wellness Center.

Health Screening Program

Program Dates: March 1 - September 30

The City Health Screening Program is voluntary and consists of completion of a biometric screening (which includes a blood draw, height, weight, and blood pressure) to earn the health plan incentive. Employees and, if applicable, spouses on the City’s health insurance plan will be assigned a month to complete their appointment at the Waukesha Employee Health & Wellness Center based on the first letter of the employee's last name.
