WHO:
Voluntary for employees and, if applicable, spouses enrolled in the City’s health insurance plan.1
WHAT:
Complete a biometric screening which includes a blood draw + physical measurements, to earn health plan incentives2 for next year.
WHEN / WHERE:
During assigned screening month, based on the first letter of employee’s last name, at the Waukesha Employee Health & Wellness Center.
Note: Enrolled spouse’s complete in same month as employee; must register and schedule separately.
HOW:

[1] Newly hired employees and employees enrolling for the first time due to a qualifying event should contact Human Resources.
[2] Costs for the health plan premium incentive will be announced during open enrollment.
