What is the Everside Health patient portal and how does it work?
Everside Health’s patient portal is a secure, convenient, and easy way to manage your health anytime, anywhere. It is available:
online at members.eversidehealth.com, or
via the Everside Health mobile app available for download in the App Store or Google Play.
With the Everside Health patient portal, you can:
Schedule appointments (virtual or in-person)
Message your care team
Join a virtual visit with your provider
Request prescription refills
Securely access your health records, including immunization records
How do I sign up?
Note: You cannot sign up until your medical plan insurance is effective.
Go to members.eversidehealth.com or download and open the Everside Health mobile app
Click 'Register by online patient account'
Enter the following information to verify your identity:
your last name,
date of birth,
email address (For a spouse or dependent the email needs to be different from the employee’s), and
the last 4 digits of your SSN (For a spouse or dependent, it’s their own SSN, not the employee’s)
Please Note: You may also be asked to enter the insurance ID number from your UHC card
Follow the remaining steps to create your password and submit

Quick Tips:
To schedule a new appointment, on the home page click Schedule appointment. From here, you can select the type of appointment you want. Once you select your appointment type, you can add any notes you want to share with your care team and press Continue, you can select whether you want an in person, phone, or video visit and you will then be prompted to choose a date and time. If you do not see an appointment that works for you, please call (262) 896-8420 to see if they can accomodate you.

To change an upcoming appointment, on the home page click on your upcoming appointment. If you would like to reschedule, select Reschedule appointment and you will be prompted to choose a new date and time for your appointment. If you would like to cancel, choose Cancel appointment.
To send a secure message to the WEH&WC care team, on the home page click Send a message.
To request a prescription refill, from the home screen, click on Health Records and then Prescriptions. If on the mobile app, you can also simply click on Prescriptions on your home screen. If on the web version of the patient portal, click on the Refill Prescriptions button.
To view lab results, from your home screen click on Health Records and then Lab Reports. You will be able to see any lab test results your provider has ordered here.
To update your phone number to ensure the care team can contact you when needed, from the home screen, click on My Profile and click the Edit button. Update any information that needs to be updated including language, address, phone number, and email. Click the Save button when complete.
To change notification settings for secure messaging and appointment reminders, from the home screen, click on My Settings and then on Notifications. When you are in the Notifications section, you can update your notification preferences. Note: Enabling email, text and/or push notifications for appointment reminders and communications, as well as direct messaging is recommended.
