If you would like to schedule an appointment with a WEH&WC Provider for an evaluation to determine if Physical Therapy is appropriate for you, you can schedule an appointment with a WEH&WC Provider through either of the above numbers as well.

Did You Know?

  • You can self-refer to Physical Therapy without a referral.

  • You can be referred to PT by a WEH&WC Provider.

  • You can bring an outside referral to receive PT at the WEH&WC.

PT Referrals Accepted from Outside Providers

Did you know that you can receive physical therapy services by self-referral or with a referral from an outside provider?

If you have a referral from an outside provider, here's what you can expect:

  • You will be asked to fax your referral to the WEH&WC prior to your appointment or bring it with you to your first visit.

  • The first visit will be an hour long one-on-one session with the Physical Therapist.

  • Following your initial visit, the Physical Therapist may request additional records to assist in your treatment.

Physical Therapist

Jen La Plante, PT, MPT

Jen earned a Bachelor of Science and Master of Physical Therapy degree from the University of Wisconsin-La Crosse. She has been practicing for over 12 years.

Physical therapists are highly-educated, licensed health care professionals who can help patients reduce pain and improve or restore mobility - in many cases without expensive surgery and often reducing the need for long-term use of prescription medications and their side effects. They can teach patients how to prevent or manage their condition so that they will achieve long-term health benefits.

They examine each individual and develop a plan, using treatment techniques to promote the ability to move, reduce pain, restore function, and prevent disability. In addition, they will work with individuals to prevent the loss of mobility before it occurs by developing fitness and wellness-oriented programs for healthier and more active lifestyles.

  • Functional strengthening

  • Joint and soft tissue mobilization

  • Manual therapy techniques

Referrals

Referrals can be made for TMJ treatment, pediatric physical therapy and other specialized areas of Physical Therapy.
