  1. Schedule your first appointment, for either of the following:

  • Any current health care needs, such as an annual physical, illness, allergies, minor injury, chronic condition, physical therapy, health coaching, and more

-or-

  • A new patient appointment if you do not have any specific needs but would like to meet with a provider to establish care


  1. Prepare for your first appointment and arrive early:

Please plan to arrive to your appointment at least 10 minutes early to check-in and update medical records. To maintain short wait times for all patients, we may reschedule your appointment if you are more than 10 minutes late.

Bring the following to your first appointment:

  • Your driver’s license and medical insurance member card

  • Your current prescription list

  • Name and contact information of PCP (optional) - if you have an outside provider and would like to share your visit information, the WEH&WC can send a full summary with your authorization

Examples of why you might establish care with the Waukesha Employee Health & Wellness Center:

  • You do not have a primary care provider and would like the Waukesha Employee Health & Wellness Center care team to meet that need

  • You have a primary care provider, and would like to take advantage of the convenience and cost of the Waukesha Employee Health & Wellness Center services for minor or acute health care needs, or when you can't be seen quickly by your own provider
